Your Policy

  • An insurance policy is a contract between you and the insurance company.
  • We cannot guarantee that your insurance will pay your claim.  We suggest you check with your employer, human resource department or insurance agent about coverage concerns.
  • You should check with your insurance company about authorization requirements prior to services provided or started.  You are responsible for payment of services not authorized or deemed necessary by your insurance company.
  • SMSI will file claims to your insurance company.  Please supply the necessary policy information at the time of service.
  • SMSI accepts insurance assignment directly from your insurance company.
  • To assist with payment, SMSI accepts personal checks, cash, Care Credit, Visa and Mastercard credit/debit cards.

If you have a question on coverage for a specific service at SMSI, please contact our office for further assistance.  We also strongly suggest that you contact your insurance provider (generally information is listed on the back of your insurance card) to verify if those services are covered under your specific contract.  Each insurance contract/policy can have different specifications.  We submit claims to the insurance plan that you provide to us at the time of your services.
